Whole Child Profile

Here’s how it works:

• Click on the "Create Your Profile" button below.
• Answer some questions about your children and family. This is your family's Whole Child Profile.
• Our system matches your needs to community services and give you a list of providers to choose from.
• The services you choose create your family’s confidential Whole Child Plan.

We urge every parent to complete the profile. With lots of service providers in our community, there may be resources or services you did not even know were available. Filling out the profile could alert you to them.

Any information you submit is strictly confidential; it will not be shared unless you give permission.
